Is your patient eligible for funding?
Patients with a clinically assessed need may receive VitalCALL free of charge under DVA's Rehabilitation Appliances Program (RAP). To receive DVA funding, your patient will first need a referral from a healthcare professional and a D9199 form completed during an in-home OT assessment. Patients may also be eligible under the NDIS, CHSP (Commonwealth Home Support Program), and state schemes.
